Specials manufacturers supplying pre-printed labels for endorsement

It has come to our attention that some specials manufacturers/suppliers are providing sticky labels to assist with prescription endorsement. However, the NHS Business Services Authority (NHSBSA) has requested to pharmacy contractors not to use these labels for endorsing purposes.

Class 4 Medicines Defect Information - Primidone SERB tablets 50mg

A medicines defect alert has been issued on Primidone SERB tablets 50mg due to an error with the EAN barcode on the packaging. When the incorrect barcode is scanned, the product is identified as Primidone SERB tablets 250mg and not 50mg.

Dispensing Health in Later Life

Patient safety alert - support to reduce the risk of death from inappropriate naloxone use

NHS England has issued a Stage Two patient safety alert on 26 October 2015 to highlight the resources available to support all NHS funded care providers on the use of naloxone. These resources can provide assistance in ensuring their local procedures and training reflect best practice on administering, dispensing and prescribing naloxone. This Stage Two alert follows on from the Stage One patient safety alert issued on 20 November 2014.

Company led recall - Bravelle powder for solution for injection 75IU

A company-led recall has been issued for all batches of Bravelle powder for solution for injection 75IU in the UK. This follows the discovery, during routine testing, that a small number of batches in Canada and the USA with a shelf-life of 24 months were found to have a reduced potency after 12 months.
